Delete Reject Reinstatement Submission Template (Excel)
Vendors must download and complete this spreadsheet when requesting reinstatement of blocked items. Once complete, email the spreadsheet along with supporting documentation to deca.lee.hq.mbx.reject-reinstate-item@mail.mil.
